Insurance Billing Process
As a courtesy service to you, we will file an insurance claim form with your insurance company. Since your carrier’s responsibility is to you, they often respond in a timelier manner when working directly with you. Therefore, we encourage you to take an active role in calling your insurance agent or carrier to get payment on your claims.
Policy on Filing Insurance Claims
We will file your primary and secondary insurance claims for you. At your request, we will provide you with a completed billing form that you may use to file your own claims with any additional carriers you may have. If you need copies of your explanation of benefits, you will need to contact your insurance carrier.
Deductibles, Co-pays and other Non-Covered Services. (Anything not covered by your primary/secondary carriers.)
We require that all deductibles, co-payments, and other non-covered services be paid by you at the time of your visit. We will add a billing fee of $5.00 if you do not pay your co-payment at the time of service.
Benefits and Eligibility
Please do not expect us to know the terms, conditions and individual coverage limits of your insurance policy. If you have questions regarding benefits and eligibility please contact your insurance carrier to determine what your benefits are. This includes verifications as to whether one of our doctors is a contracted provider with your insurance carrier. Please note that you are ultimately responsible for payment for all services that are not covered by your insurance plan.
Payment Options
We accept cash, checks, VISA, MasterCard, and American Express.
Please Note...
We will bill your insurance as a courtesy to you. Services not covered or reimbursed by your insurance will be your responsibility. We require monthly payments of $50.00 or 10% of your balance, whichever is higher.
We do accept cash patients. If a patient with no insurance pays at the time of visit they will receive a 20% discount. Patients that are unable to pay the entire bill are required to pay a down payment.
